When You Can Apply
• Applications are accepted only during the 3rd Monday of each month.
• We are only accepting 25 applications.
• Completed Applications are processed on a first-come, first-served basis.
How the Process Works
• Pick-up application before the 3rd Monday of each month
• Drop off application in person on given date. (Call for date)
• All applications are reviewed and scheduled later if qualified.
• Appointments are NOT given when you drop off the application.

Priority Groups
Applications are prioritized for:
• Working Families
• Seniors aged 60+
Required Forms (print required)
The application forms are available to download below. Please print and complete the forms and return them in person to our office with ALL required documents (listed below) on the 3rd Monday of the current month. (Call for turn-in date) Forms cannot be submitted by email.
