Intensive Housing Case Manager

Overview

Responsible for providing comprehensive services and information that enables participants to take an active role to secure housing.  The Housing Case Manager collaborates with participants to identify, define, and establish individual housing goals and maintain secure housing; acts as an advocate to participants by assisting them with the development of a housing plan and provides assistance to remove barrier for maintaining secure housing. Responsible for developing a housing stability plan and ensures clients are progressing to meet stability goals.  Provides assistance in making referrals for education and employment. Assists to provide life skills and meets with community resources on behave of the client as needed. Responsible for developing relationships with community housing resources for client placement. Responsible for working in a team environment while advocating for participant housing goals and establishing relationships with community partners to provide additional resources as needed.

Responsibilities

  • Conducts orientation and client assessments.
  • Assists all clients in housing search, barrier resolution, and a plan to provide secure housing.
  • Maintains client records and enters data in real time in HMIS.
  • Ensures that clients adhere program requirements.
  • Meets with clients weekly to provide case management as needed.
  • Assist clients in exploring employment opportunities; provides life skills guidance, coaching and support.
  • Assist clients in reviewing lease terms and conditions.
  • Assist clients in resolving conflicts with landlords to keep secure housing.
  • Assist clients in the development of a housing plan to include employment and budget review to retain secure housing.
  • Support clients through all stages of the program, addressing any obstacles or challenges faced by the client and make referrals as necessary.
  • Develops and monitors client housing plans.
  • Assists to provide supportive services for participants to remove barriers for housing and employment when funds are available through AIM and other community agencies.
  • Inputs data into data systems and assists in tracking all data requirements in real time for data management of services/assistance provided to participants.
  • Completes other duties as assigned.

Reporting Structure

This position will report to the Director of Housing. This is a full-time position.

Qualifications

  • Bachelors level degree from an accredited college or equivalent experience, preferably in housing programs, workforce or similar services.
  • Experience providing intensive case management preferred.
  • Excellent organizational and interpersonal skills are required.
  • Must have strong communication, writing, and computer skills.
  • Ability to work with low income job seekers; proven ability to work as part of a team; ability to multi-task, work independently, prioritize and meet deadlines.
  • Must have ability to interact effectively, both internally and externally to Anderson Interfaith Ministries, with people who are diverse in skills, cultures, and authority levels, interests, backgrounds, and economic status, to get tasks accomplished.
  • Must be able to work well with minimal supervision and off-site office arrangement where clear and consistent communication is imperative.
  • Proficient in Microsoft Word and Excel.
